Referrals

For patients referred by an ENT specialist

If you have been referred from a private ENT clinic, please select the appropriate service required on the booking page. Please bring your referral letter and any relevant test results with you if they are available to you. If you are unsure which appointment to choose, contact CluasCare before booking.

GP and other professional referrals

Referrals from GPs and other healthcare professionals are welcome. Please include the reason for referral, relevant history, any previous test results and the assessment requested. Please email your referral to cluascare@gmail.com.

Booking your appointment

Bookings can be made through the SumUp booking page at www.sumupbookings.com/cluascare. If your referral specifies an appointment type, select that option on the booking page.
